(Prepay with this form.)  Paint will be $55 or $60/case depending on brand.  BYOP $20.  Enclose air fees and BYOP fees with this form.  You may wait to purchase paint at the event.  DEADLINE: This registration must be postmarked by Nov. 18 for free entry to the event to apply.

   INSURANCE WAIVERS: Please help us make things run smoothly by having an insurance waiver signed by each person before you check in. Waivers are REQUIRED and can be found at our web site. One  waiver  per player is required.  Minors MUST have their waiver signed by their parent or legal guardian.
     PAINT SALES:  You may purchase our paint at the event, pre-pay for both air and paint, or BYOP for an additional $20 or you may BYOP if you also purchase a minimum of 1/2 case of event paint.  Make out all money orders / checks / cashier checks to Smak Zone Paintball.  Note: All BYOP paint is subject to a safety check.
     FORFEITS: Smak Zone events are held rain or shine.  There will be no refund of payments except for unavoidable circumstances.  (Documentation to the nature of the emergency required.)  If you have to cancel after you have made payment, contact Smak Zone Paintball to submit a request for refund.  Any refunds agreed to will be paid within 60 days after game date.  Otherwise we will credit you the cash value toward a future game.
     ALCHOHOL: Booze and paintball don't mix. NO ALCHOHOLIC BEVERAGES ALLOWED ON SMAK ZONE PAINTBALL PREMISES!  If you have to have it, look at the local lodging page for other places to camp.
     SPECIAL ROLES: If you would like to play a special or specific role, i.e. tank commander, artillery operator, ect., please make a note on this form and contact us.  While encouraged, all special weapons or vehicles, (LAWS, cannons, mortars, mines, tanks, ect.) must pass rigid safety checks.  If you have any questions, e-mail mwm@smakzone.com or phone 573-856-4009 or visit  www.smakzone.com
